Case Management

Case managers assist people living with HIV to assess ongoing needs, develop a plan to meet these needs and connect people to community resources. In order to optimize a clients ability to manage HIV, case managers assist in coordination of health care, provide education about HIV and advocate to meet basic living needs. Case management services are provided for HIV-positive people in the Twin Cities metro area and counties in and around Duluth.

Case managers can help clients:

  • Gain access to needed health care resources including referrals to physicians specializing in HIV
  • Get information needed to make informed health decisions
  • Learn about medication adherence, safer sex and other risk reduction techniques and health-enhancing behaviors
  • Access needed resources such as support groups, chemical dependency treatment facilities, legal assistance, vocational rehabilitation services and mental health resources
  • Plan budgets and access applications to disability and insurance programs
  • Develop a supportive network, whether through friends, family or a support group, that clients can rely on when needed
