Transportation Services

MAP provides transportation services to eligible HIV-positive individuals living in the Twin Cities Metropolitan area for medical and pharmacy visits.

The program provides door-to-door rides by taxicabs. Rides must be requested at least two working days in advance. Service is provided Monday through Friday, 8:00 a.m. to 5:00 p.m. (holidays excluded).

Bus passes are available for HIV-positive persons who cannot obtain a bus pass from other programs.

Eligible residents of Chisago, Isanti, Sherburne, and Wright counties in Minnesota can receive mileage reimbursement for medical and pharmacy visits.

Some food shelf pick-ups and delivery is available through volunteer drivers.

Transportation Forms

The following forms are required in order to sign up for the MAP Transportation Program in the Twin Cities Metro Area.

• Metro Area Transportation Guidelines (85KB PDF) – Explains how to use the Transportation Program, what types of services the program provides and how they are provided. All clients should read and keep this form for their reference.
• HIV Verification Form (62KB PDF) – Must be completed and signed by the client's medical doctor and given to us during the application process. We can accept other proof of HIV diagnosis, such as a letter from a doctor or lab report which states that the client is HIV positive, instead of this form.
• Data Practice Notification Form (96KB PDF) – Explains how we will treat a client's personal information. The client should read, sign and return this form.
• Transportation Application Form (115KB PDF) – Complete both pages. Client should sign and return the second page with proof of current income. Income verification must be updated annually.
• Authorization to Exchange Information Form (83KB PDF) – Must be completed and signed by the client if they want the MAP Transportation Program to be allowed to speak with their clinic, case manager, family member, partner or any other person about their services. This form must be updated annually.
• Minnesota AIDS Project Client Bill of Rights Form (52KB PDF) – Explains the client's rights. The client should read, sign and return this form.
